IUD or Nexplanon) are also included in your monthly fee. (There may be additional charge for medications and supplies used for the procedure but there is no physician charge for the procedures).

  • Pediatric Care

    We see children from age 5 and up. Services include well child checks, sports physicals, and same and next day appointments for acute issues.

  • Women's Health

    We offer a full range of women's health services, including PAPs, breast exams, menopausal consults, STD screening, contraceptive consultation, and more.

  • Men's Health

    We provide evaluation for fatigue and management, prostate health, testosterone testing and low T management, sleep disorders and more.

  • Chronic Disease Management

    With unrushed appointments, we have the time to focus on preventative health, improving and managing chronic diseases. We value the importance of lifestyle changes on chronic disease management.

  • Acute and Urgent Care

    We provide same or next day appointments with easy access to your doctor. No more waiting rooms.

    We have several in office testing that are included in membership ex: urine pregnancy testing, urinalysis, EKG, breathing treatment and more.

  • Discounts:

    • If you pay for 11 months up front, you get 12th month free

    • $10 OFF monthly for spouse/partner

    • If >3 children enrolled, cost is $60/month for 3rd child

  • Children (less than 16years old) must have parent or guardian enrolled.

  • One Time Enrollment Fee: Individual ($50), Couple ($90), Family ($120)

  • Our direct primary care members receive discounts on any service not included in the membership.

Discounted Services

Our members have access to several primary care services that are heavily discounted and charged at wholesale cost. As a member, you pay what we pay for these services. We have negotiated discounted cash pricing for some services that occur outside of our clinic. For our members with health insurance, we can bill these services to your insurance if you prefer.

There are some services not included in your monthly membership fee. We work with patients to get affordable pricing for some of these services : ex. PT, pathology fees, imaging studies and labs. Upfront Pricing is Provided for most services not included.

Immunizations - we cannot, at this time, perform immunizations due to cost. Immunizations can be received at local pharmacies or the health department. We have tentative plans to add a few immunizations Fall 2024 (Influenza and Tdap). At this time, all our childhood immunizations are referred to the Health Department.

FAQs

  • Medicare and Medicaid

    • We accept all patients however we do not bill Medicaid, Medicare or any other private insurance. All members follow our pricing plans.

  • Obstetrics

    • As a family physician, we can continue to see you during pregnancy for many issues however, we require our pregnant patients to see an OB/GYN team for pregnancy care and childbirth.

  • Therapy and Long Term Care

    • Our members referred to therapy (physical, behavioral health, speech etc) are provided with upfront expected pricing.

  • Specialist or Hospital Care

    • Our membership does not include ER visits, hospitalization, specialist or surgical care coverage. We can refer to any specialist or hospital of your choice and we work with your team for updates during your care. Our members have access to follow up visits within 3-5days of discharge and as usual can call, text or email us with any concerns.
